First Time Visit

Understanding the basics of your first visit and being prepared with the necessary information and documentation will allow us to better assist you and eliminate any extra wait time. Your initial appointment will consist of a consultation with our team along with a diagnosis and available treatment options.

Surgery will then be scheduled for another appointment, though in some cases of more minor procedures, we can occasionally perform the surgery on the same day as your consultation. The time in between your consultation and your scheduled procedure will give our staff time to contact your insurance company and make the necessary filings.

If our patients prefer us to not contact their insurance company, or if they do not have applicable insurance coverage, payment for the full balance of the treatment is expected prior to any surgical treatment being rendered.

At the time of your consultation, please be prepared to give us the following:

  • A surgical referral slip and any X-rays if applicable
  • A listing of any medications you are presently taking
  • If you have applicable medical or dental insurance, please bring the necessary completed forms. This will save time and allow our office staff to help you process any insurance claims.

Important Patient Information

All patients under the age of 18 must be accompanied by a parent or guardian at the consultation visit, and during the surgical procedure.

Procedure Requiring IV Anesthesia:

For all patients undergoing procedures requiring IV Anesthesia, a pre-operative consultation and physical examination is mandatory. Please refrain from eating or drinking for 6 hours before your surgery, unless you’ve received other prior instructions from you doctor. You will also need a licensed driver to drive you home following the surgery.

Please alert our office if you have any medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) and of any medications you are currently taking (i.e. heart medications, aspirin, anticoagulant therapy, etc.).

X-Rays

Please request that your dentist or physician forward any x-rays to our office in advance of your procedure. If conditions prohibit your dentist or physician from being able to forward the x-rays to us before your procedure, please pick them up and bring them to our office. If one of our doctors feel that additional x-rays are necessary, they can be taken at our office.

Surgical Instructions

You may preregister with our office by filling out our secure online Patient Registration Form. After you have completed the form, please make sure to press the Submit button at the bottom to automatically send us your information. On your first visit to our office, we will have your completed form available for your signature. The security and privacy of your personal data is one of our primary concerns and we have taken every precaution to protect it.
